Neviraipne for PMTCT
Could someone out there please enlighten me on the use of Nevirapine only for a prolonged period of time in PMTCT when even a single dose causes resistance?

Hi,
Single-dose nevirapine is the main option for the prevention of mother-to-child transmission (PMTCT) of HIV-1 in countries with limited resources, during active labour. As the name suggests, it is just one single dose and is not used for prolonged periods alone.
However, the use of single-dose NVP may result in resistance which could compromise the success of subsequent treatment of mother and child with antiretroviral combinations. Around a third of women who take single dose nevirapine develop drug resistant HIV, which can make subsequent treatment involving nevirapine and efavirenz (a related drug) less effective later.
Usually the mother is given antiretrovirals (AZT and 3TC) starting from the 14th week itself and an additional single-dose of nevirapine is given during active labour.
Is anyone using mono therapy with nevirapine for long durations as you are suggesting?
