What is difference between acute and chronic rheumatic heart disease?

Acute means "recent onset, shorter duration" and chronic means "long duration" disease. As far these terms in rheumatic heart diseases, I would redirect you to this post: Rheumatic Fever and Rheumatic Heart Disease.
