    Hi,

CAD means coronary artery disease. Coronaries are the arteries that supply blood to heart wall muscles. A block in them is responsible for the heart attack (myocardial infarction).

LV means left ventricle. It is the chamber of the heart responsible for pumping blood to various body organs. The wall muscles of that chamber are not functioning well as per the report.

Refractory cardiac failure is when the LV dysfunction has not significantly responded to the usual modes of medical therapy.

    Summary:
What the patient has is an abnormality of heart function without any significant block in the coronary arteries. This is not myocardial infarction, but a lower grade of that condition.
Since the coronary arteries are normal, angioplasty or bypass surgery are not required and the patient needs medical management with regular medications and follow-up.
