    approach to a 7year old child having convulsions

    a 7 yr old patient was seen having convulsions followed by unconsciousness for the last 16 days.the patient was afebrile before convulsions started.he did not have prior history of drug intake or trauma.
    the patient has repeated episodes of convulsions of the whole body followed by unconsciousness.
    Can you give any idea about the cause of such convulsions and its management.

    This patient with repeated episodes of convulsions followed by unconsciousness for more than 2 weeks seems to have had status epilepticus/ recurrent seizures (depending on the details of initial history) with hypoxic brain injury.

    Some additional clinical features and basic investigations will provide better clue.
    1. neurological examination - any focal deficits/ pupil abnormalities?
    2. basic blood counts and biochemistry
    3. CT scan of brain
    4. Lumbar puncture with CSF analysis, if done.

    If the patient still has repeated seizures, metabolic causes like electrolyte imbalances should first be looked for and corrected. Combinations of antiepileptic therapy (by adding newer drugs as adjuncts) is warranted if the patient is not already on these. Please provide some more details, and we can discuss.
